An ultrasound was recommended which led to the identification of the condition. The clinical evaluation revealed right kidney cancer with tumour thrombus extending into the IVC, a large vein that carries blood from the lower body to the heart, and, unusually, involvement of both renal veins. This pattern of spread is extremely rare and presents significant surgical and clinical challenges.<br \/>\nA multidisciplinary team led by Dr. Antan Uresh Kumar T, Senior Consultant, Urology, undertook a highly complex surgery lasting nearly eight hours. The procedure involved removal of the affected right kidney along with meticulous clearance of tumour from the IVC and both renal veins, while preserving the function and integrity of the left kidney.<br \/>\n\u201cThis was a highly complex case because the tumour had extended into the inferior vena cava as well as both renal veins, which is extremely uncommon. The key challenge was to achieve complete tumour clearance while preserving the function of the unaffected kidney. Such cases require careful planning and close coordination across specialties. The patient\u2019s recovery and stability post-surgery are encouraging, and she continues to be under structured follow-up and treatment,\u201d said Dr. Antan Uresh Kumar T, Senior Consultant, Urology.<br \/>\nThe involvement of both renal veins alongside the IVC makes this case particularly uncommon, especially in the context of a Ewing\u2019s sarcoma variant of kidney cancer. Managing such extensive vascular involvement requires close coordination across specialties and precise intraoperative decision-making to balance complete tumour removal with organ preservation.<br \/>\n\u201cCases like these highlight how kidney cancers can sometimes progress silently until they involve critical vascular structures. Early evaluation of symptoms such as persistent flank pain or unexplained swelling is important. At the same time, the ability to manage such complex presentations through a multidisciplinary approach is central to improving outcomes in advanced cancers,\u201d he added.<br \/>\nThe surgery was performed with support from a multidisciplinary team including Dr. Jayakrishnan, Senior Consultant, Vascular Surgery; Dr. Nikhil, Senior Consultant, CTVS; Dr. Suresh, Senior Consultant, Medical Oncology; Dr. Mohanakrishnan, Consultant, Pulmonology; Dr. Ravikumar, Senior Consultant, Nephrology; and Dr. Muthukumaran, Senior Consultant, Medical Gastroenterology.<br \/>\nFollowing surgery, the patient had a stable recovery and was discharged within a week.
